Cloud Awards Nominations are now closed for 2023.
The Arizona Partnership for Immunization (TAPI) sought nominations for Teen and Toddler awards. These awards recognize the exceptional efforts of the many individuals and organizations whose tireless work and innovative strategies have improved immunization coverage levels statewide. We look forward to our annual awards celebration as an opportunity to recognize immunization efforts over the past year.

Vaccine Series by Age Group
Each Cloud Award recognizes the recipient for 2 full years. However, practices are able to apply to the award of their choice for 2024 (even if it was received the previous year.)
In order to be an award winner, you must be top performing in your practice for coverage levels for toddler and/or teen award(s) for the following series:
*Note that a minimum of 20 active patients in the age group is required to win an award. Practices with less than this number who are top performing in their practice coverage level can qualify for an Honorable Mention Award.
