ABOUT CLOUD AWARDS

The Arizona Partnership for Immunization (TAPI) seeks nominations for Toddler and Teen Dr. Daniel T. Cloud Awards, opening January 25, 2024, and closing Thursday, March 14, 2024.

What is a Cloud Award: The Dr. Daniel T. Cloud Awards recognize the exceptional efforts of the many individuals and organizations whose tireless work and innovative strategies have improved immunization coverage levels for toddlers and teens in Arizona.

Who can submit a nomination: Anyone may submit a nomination. You may nominate your own practice.

Who can be nominated: Any organization that vaccinates Toddlers (24-35 months) and/or Teens (13-15 years.) The organization may or may not be VFC providers.

Winner Criteria: Award winners are determined by being top performing practices based on 90% coverage for specific toddler and/or teen vaccine series.

Vaccine Series by Age Group

Each Cloud Award recognizes the recipient for two full years. However, practices may apply for the award of their choice for 2024 (even if it was received the previous year.)

In order to be an award winner, you must be top performing in your practice for coverage levels for toddler and/or teen award(s) for the following series:

Sample ASIIS Reports

Toddler Reports, please be sure:

  • You select series: 4:3:1:3:3:1:4 (4 DTaP, 3 Polio, 1 MMR, 3 Hep B, 3 Hib, 1 Varicella and 4 Pneumo [PCV])
  • The “Age Range” on your report shows “24 months through 35 months”
  • The “Evaluate At Age” on my report shows “24 months”

Teen reports, please be sure:

  • You select Series: 1 TDAP, 1 MCV, UTD HPV
  • The “Age Range” on your report shows “13 years through 15 years”
  • The “Evaluate At Age” on your reports shows “All”

 

Please check these before submitting to make sure your nomination is complete and your reports are correct. Inaccurate reports will need to be re-done and can delay the review process.

Didn’t reach 90% or more on your first report(s)? You could still be a Cloud Winner! Follow these instructions and resubmit your report(s) before the deadline to see if you qualify.

Instructions for Inactivating Patients

CLICK HERE for step-by-step instructions for inactivating patients who have moved away or gone to another practice so no longer appear as your patients in ASIIS.

 

Which Children Should NOT be Included in Assessments

  • Flu/Covid Clinics: You can and probably should inactivate children ONLY vaccinated at your flu/covid clinics. Clinics should NOT have been assigned ownership for children in mass clinics.
  • Community Catch-Up Clinics: If you are an FQHC who did community catch-up clinics, those children seen only at the catch-up clinics should be inactivated.

If you see children for well visits, they belong in the assessment. If not, they don’t.

 

Families Refusing Vaccinations

If your clinic has children who got most of the series from your clinic, even if they refuse to come back, they still count as your patients. If you have a child whose parent opts out of a vaccine like MMR, you should have them sign a Refusal to Vaccinate Form (English: https://downloads.aap.org/DOPCSP/SOID_RTV_form_01-2019_English.pdf, Spanish: https://downloads.aap.org/DOPCSP/SOID_RTV_form_01-2019_Spanish.pdf)

Winners will be notified about how to claim free tickets. One free ticket will available per award winner. Even if you won Cloud awards for both Toddler and Teen, you will only receive one free ticket.
