What is a Cloud Award?
The Dr. Daniel T. Cloud Awards recognize the exceptional efforts of the many individuals and organizations whose tireless work and innovative strategies have improved immunization coverage levels for toddlers and teens in Arizona.
Who can submit a nomination?
Anyone may submit a nomination. You may nominate your own practice.
Who can be nominated?
Any organization that vaccinates toddlers (24-35 months) and/or teens (13-15 years.) The organization may or may not be VFC providers.
Winner Criteria
Award winners are determined by being top-performing practices based on 90% coverage for specific toddler and/or teen vaccine series.
